Comprehensive accident investigations are critical to preventing accidents from re-occuring.  Yet most accident investigation reports that I review are incomplete and/or poorly done.

Improve the quality of your accident investigations by holding your investigators accountable for their accident investigations, and then evaluating and rating their accident investigation reports.
